CHI’s online submission form is an option for families that have completed their social worker written reports and are writing their own post-adoption reports on their child. You will automatically receive a confirmation email when the report is successfully submitted.
If you are a Social Worker submitting a notarized report on behalf of the family, please mail the report to CHI’s Washington office.
